PPSMC Consultation Services

Palliative Pain and Symptom Management Consultants are unique in that they can be consulted by any provider (including, Personal or Direct Support Workers, Nurses, Physicians, Volunteers, Patients or Caregivers etc.) to support the core team of health care providers in caring for people with progressive life-limiting or life-threatening illnesses.


  • PPSMCs do not require a physician referral or physician consent- but we do require the consent of the patient or their substitute decision maker
  • PPSMCs are organizationally neutral- meaning we go anywhere in any setting to meet the patient and the provider where they are
  • There is no cost to the requesting organization or provider and we DO NOT bill OHIP

Consultation Support from a PPSMC can be any of the following:

  • Difficult conversations
  • Advanced Care or End Of Life care planning
  • Symptom assessment and management
  • Care approaches (trauma-informed, dignity-conserving)



Consultation can be provided by the following means: in-person, virtual or phone.
