Palliative Pain and Symptom Management Consultation (PPSMC)

Ontario's PPSMCs are nurses with advanced palliative care knowledge who educate and advise service providers on best practice in palliative care. The program is fully funded by Ontario Health.

Their mandate: consultation, coordination, education and skill development, research and evaluation, and collaboration and facilitation. That includes mentoring providers, connecting them to resources, consulting on specific cases, and supporting system improvement through committees.

Who can be referred: anyone with a progressive, incurable illness whose symptoms aren't well controlled by their core team. Appointments are in person, by phone or virtual. We review symptoms and advance care planning, get to know the person and their goals, may do joint visits, and then work with your health care team afterwards. Designed to align with the Model to Guide Hospice Palliative Care: Based on National Principles and Norms of Practice and The Ontario Palliative Care Competency Framework.

Essential Pain Management (EPM) Program

Focuses on Pharmacological Management of Pain. Pain assessment and management are essential skills for nurses in any setting. EPM aims to equip nurses with foundational knowledge and skills for pain management, applying a palliative care approach in practice.

Advanced Palliative Practice Skills (APPS) Program

An educational opportunity for Personal Support Workers (PSWs), Health Care Aides and Hospice Volunteers who are graduates of the Core Fundamentals of Hospice Palliative Care Program and who are currently working or volunteering in palliative care.

Learning Essential Approaches in Palliative Care (LEAP) Program

Offers courses designed to equip healthcare professionals with essential skills for effective palliative care. Created by Pallium Canada, these courses focus on communication, symptom management, ethical decision-making, and support for patients and families dealing with life-limiting illnesses.

Canadian Serious Illness Conversation (CSIC) Training

Canadian Serious Illness Conversation (CSIC) training strengthens health care professional communication skills in compassionate and effective conversations with patients, families, and caregivers facing serious illness.
